Membership within The American Foundation for Donation & Transplantation (AFDT)
consists of:
-
Transplant centers
- Organ procurement organizations
- Histocompatibility laboratories
- Tissue banks
- Individuals in the field
Memberships available: Institutional/Full, Associate, Individual
Institutional/Full Membership Criteria
TRANSPLANT CENTER
- Approved By UNOS
- Agreement with a Medicare Certified Organ Procurement Organization
- Agreement with a AFDT or ASHI Approved Histocompatibility Lab
- Agreement that Histocompatibility Lab participates in at least one additional Proficiency Test, which could include, but not be limited to, the AFDT Proficiency Test
- Completion of AFDT Certification Form with Curriculum Vitae of Surgeons and Transplant Physicians
- Agreement with AFDT to submit Activity Report and Data Forms
- Notification of Board Representative and Alternate
INDEPENDENT ORGAN PROCUREMENT ORGANIZATION
- Approved Medicare OPO
- Agreement with AFDT or ASHI approved Histocompatibility Lab
- Notification of Board Representative and Alternate
- Non-Profit Organization
HISTOCOMPATIBILITY LABORATORY
- Approved by AFDT or ASHI
- Agreement to participate in at least one additional Proficiency Test, which could include, but not be limited to, the AFDT Proficiency Test.
- Notification of Board Representative and Alternate
- Curriculum Vitae of Director
TISSUE BANK
- Knowledge of AFDT Guidelines for Tissue Banking
- Notification of Board Representative and Alternate
- Non-Profit Organization
- Bone & Tissue Committee also assesses an annual fee of $500 for committee activities
Associate Membership Criteria

- Approved by UNOS
